2014 Point-in-Time Count Forms

NOTICE: On January 17, HUD released an update that changed the information communities are required to collect for the Point-in-Time Count. NCCEH staff posted revised versions of the following forms on January 21: Sheltered Homeless Count Form, Unsheltered Homeless Count Form, CoC Homeless Count Form.

If you downloaded one of the affected forms prior to January 17, please DO NOT use it to report your data for the Point-in-Time Count. Please use the new versions posted below.

The 2014 North Carolina Point-in-Time Count will be held the night of Wednesday, January 29, 2014. Please use the forms below to record and report your data from the count.

Sheltered Homeless Count Form

This form is for emergency shelters (including domestic violence shelters), seasonal shelters, and transitional housing programs serving homeless people.
  • Programs in the Balance of State CoC must turn the forms in to NCCEH by Feb. 14.
  • Programs in all other CoCs should turn the forms in to their local PIT coordinator.

Unsheltered Homeless (Street) Count Form

This form is for communities conducting street counts or services-based counts of unsheltered homeless persons.
  • Communities in the Balance of State CoC must turn the forms in to NCCEH by Feb. 14.
  • Communities in all other CoCs should turn the forms in to their local PIT coordinator.

Permanent Housing Count Form

This form is for rapid re-housing and permanent supportive housing programs.
  • Communities in the Balance of State CoC must turn the forms in to NCCEH by Feb. 14.

Homeless Demographic and Needs Survey

This is an optional survey that can be used while interviewing homeless persons during the count to collect information that must be included on the reporting forms.
  • These forms do not need to be turned in to NCCEH.  Check with your local PIT coordinator to see if forms are being collected locally.
  • You may edit the Word version of this form according to your community's needs.

Continnum of Care Reporting Forms

These forms are for Continuum of Care leadership to report the CoC's total count to NCCEH. 
  • Communities in the Balance of State CoC do not need to submit these forms to NCCEH.
  • All other CoCs in North Carolina must submit these forms to NCCEH by March 7.

Imminently Literally Homeless/At-Risk of Homelessness Form

This is an optional form is for communities that wish to collect data on people who are imminently literally homeless or at risk of homelessness.
  • This form does not need to be submitted to NCCEH. Check with your local PIT coordinator to see if forms are being collected locally.
