Each year, the Scorecard Committee creates scorecards that are used by the Project Review Committee to score Project Applications.

Each Regional Committee that has new or renewal applications must fill out this form and return it to NCCEH by December 31, 2012. Applicants must present their projects to their Regional Committee for approval and priority ranking. This process may be conducted in person or via email.

All agencies that are considering applying for new or renewal funding should complete this form.
Pre-Application Form: Part One (THIS FORM IS NOW CLOSED)
All agencies who are requesting funds for new projects in the 2012 CoC funding application are required to complete a pre-application. (This is NOT for renewal projects.)
This form must be completed online. This is the first step in the 2012 BoS CoC pre-application process. You will need to complete the Narrative Questions and Rental Assistance Worksheets below before submitting this online form.
Each applicant must complete this document and upload it when completing the Online Pre-Application Form.
If you are requesting rental assistance, you must complete this excel spreadsheet and upload it when completing the Online Pre-Application Form.
