Ending Homelessness Through Systems Change: Lessons from the Taking Health Care Home Evaluation
The Corporation for Supportive Housing has released an evaluation summary outlining the scope and impact of the strategies used in the national Taking Health Care Home (THCH) initiative. Launched in 2003 thanks to funding provided by the Robert Wood Johnson Foundation,THCH is designed to prevent and end long-term homelessness for people with chronic health conditions by changing the way that housing, health and social support services are financed and organized. While the report itself focuses primarily on outcomes from the initiative, the summary of this report is a great guide to the science and art of creating meaningful system change.
