7.20.12 Toll-Free ACH Complaint Number This is the number to call with any complaints about an ACH facility. This includes concerns regarding the current issue of IMD determination and its impact on the residents of the facilities. If an individual or their guardian feels that their rights have been violated, they are pressured to make decisions, or daily needs are being neglected, that individual or guardian should call and make a complaint. NOTE: The line cannot take calls for advocates, family, or others in the community on behalf of an individual. The complaint must come from the individual or the guardian. 1-800-624-3004
